    About Us

Boyup Brook Medical Services is an AGPAL accredited general practice and services the Shire of Boyup Brook and surrounding areas.

Our General Practitioner is Dr Mildred Chiwara.

Dr Chiwara is available for appointments on Monday, Tuesday, Thursdays and Fridays. 

Kelly O’Reilly is our Practice Nurse and is available Mondays, Tuesdays and Fridays. Kelly is available for Care Plans, vaccinations, wound checks and dressing changes and ear syringing.

Janette is our Practice Manager, Pia and Madison are our friendly and experienced reception staff.

    Appointments

If you would like to schedule an appointment, please call us on 9765 1026.  Consultations are generally 15 minutes. Longer appointments can be made upon request, please advise reception if you require extra time when making your appointment. Every effort will be made to accommodate your preferred time and doctor.  Please provide a minimum of 2 hours notice if you are unable to attend your appointment. Appointments made and cancelled or not attended on the same day will incur a fee of $40 which is non-refundable through Medicare.

    Fees and Charges

As a mixed billing practice, we offer bulk billing to all valid Health Care Card holders, Pension Card holders, DVA Card holders and children under 16 years with a valid Medicare Card.

Out-of-pocket costs may apply to cover consumables used during your consultation. Your doctor will inform you of these costs at your appointment. There is no Medicare rebate for consumable costs.

For private patients, fees will be charge at the discretion of your doctor. Please see below for current private billing rates as at July 2024.

  Appointment Reminders, Cancellation and Non-Attendance

A SMS is sent to you the day prior to your appointment. We ask you to kindly respond to this SMS to confirm your appointment.

Please provide a minimum of 2 hours’ notice if you are unable to attend your appointments. If an appointment is missed or reception is not notified, a non-attendance administration fee of $40.00 may apply which is non-refundable through Medicare.

  Repeat Prescriptions & On-going Referrals

The request for repeat prescriptions and on-going referrals without seeing a doctor are completed at the discretion of your Doctor.  Please note these incur a cost and there is no Medicare rebate. You can request these by calling and speaking to our friendly reception team, or if you prefer, you can see a doctor for repeat prescriptions. If you have not had an in-person consult with your doctor in the previous six months or you require a repeat prescription for a medication one of our doctors hasn’t prescribed you, you are required to make an appointment. Please bring in your old medication boxes if possible.

  Workers Compensation

Please advise reception staff if your consult is in relation to Workers Compensation. We will need to get several details from you. You will be responsible for paying for your visit on the day, unless we have written confirmation from your employer or their insurance company prior to your appointment. You can be reimbursed by the insurance company when the claim is accepted.

  Your Rights

If you have a problem, concern or suggestion we would like to hear about it. We are always looking for quality improvement opportunities and our patients are one of our best sources of information on how we perform. Please feel free to talk to any of our staff members. A complaints, compliments and suggestion box is also located next to our front door. If you wish to discuss a serious matter with an independent organisation, you may contact the Health and Disability Services Complaints Office in Perth by phone 1800 813 583.

  Your Privacy

When you register as a patient of our practice, you provide consent for our GPs and practice staff to access and use your personal information so they can provide you with the best possible healthcare. Only staff who need to view your personal information have access to your records. If your information is required for additional purposes, we will seek consent from you prior to accessing your records.

We require consent from you for another person or organisation to act on your behalf. This includes making appointments, ordering and picking up scripts and referrals and/or requesting documents on your behalf. If you would like to nominated someone to request this information on your behalf, please fill in a consent form available at reception.

  Why do we collect, use, hold and share your personal information?

Our practice will need to collect your personal information to provide healthcare services to you. Our main purpose for collecting, using, holding and sharing your personal information is to manage your healthcare. We also use it for directly related business activities, such as financial claims and payments, practice audits and accreditation, and business processes.

  What personal information do we collect?

The information we collect about you includes:

  • Names, date of birth, addresses and contact details
  • Medical information including medical history, medications, allergies, adverse events, immunisations, social history, family history and risk factors
  • Medicare number (where available) for identification and claiming purposes
  • Healthcare identifiers
  • Health fund details.

  Dealing with us anonymously

You have the right to deal with us anonymously or under a pseudonym unless it is impracticable for us to do so or unless we are required or authorised by law to only deal with identified individuals.

  How do we collect your personal information?

Our practice will collect your personal information:

  • When you make your first appointment our practice staff will collect your personal and demographic information via your registration
  • During the course of providing medical services, we may collect further personal information. This may include collection through Electronic Transfer of Prescriptions (eTP) and the MyHealth Record/PCEHR system, e.g. via a Shared Health Summary or Event Summary.
  • We may also collect your personal information when you send us an email, SMS or telephone us
  • In some circumstances personal information may also be collected from other sources. Often this is because it is not practical or reasonable to collect it from you directly. This may include information from:
    • your guardian or responsible person
    • other involved healthcare providers, such as specialists, allied health professionals, hospitals, community health services, pathology and diagnostic imaging services
    • your health fund, Medicare, or the Department of Veteran's Affairs (as necessary). 

  Who do we share your personal information with?

We sometimes share your personal information:

  • With third parties who work with our practice for business purposes, such as accreditation agencies or information technology providers – these third parties are required to comply with the Australian Privacy Principles and our practice privacy policy
  • With other healthcare providers
  • When it is required or authorised by law (e.g. court subpoenas)
  • When it is necessary to lessen or prevent a serious threat to a patient’s life, health or safety or public health or safety, or it is impractical to obtain the patient’s consent
  • To assist in locating a missing person
  • To establish, exercise or defend an equitable claim
  • For the purpose of confidential dispute resolution process
  • When there is a statutory requirement to share certain personal information (e.g. some diseases require mandatory notification)
  • During the course of providing medical services, through Electronic Transfer of Prescriptions (eTP), MyHealth Record/PCEHR system (e.g. via Shared Health Summary, Event Summary).

Only people that need to access your information will be able to do so. Other than in the course of providing medical services or as otherwise described in this policy, our practice will not share personal information with any third party without your consent.

We will not share your personal information with anyone outside Australia (unless under exceptional circumstances that are permitted by law) without your consent.

Our practice will not use your personal information for marketing any of our goods or services directly to you without your express consent. If you do consent, you may opt-out of direct marketing at any time by notifying our practice in writing.

  How do we store and protect your personal information?

Your personal information will be stored at our practice as electronic records. Our practice has excellent physical security, electronic security and system back up procedures in place.

  How can you access and correct your personal information at our Practice?

You have the right to request access to, and correction of, your personal information. Our practice acknowledges patients may request access to their medical records. We require you to put this request in writing either by letter or email reception@boyupmedical.com.au and our practice will respond within 30 days. A reasonable fee may apply.

Our practice will take reasonable steps to correct your personal information where the information is not accurate or up-to-date. From time-to-time, we will ask you to verify your personal information held by our practice is correct and up-to-date. You may also request that we correct or update your information.  Please make these requests in writing either by letter or email reception@boyupmedical.com.au 

  SMS, Email and My Health Record

This practice may use SMS and email for appointment reminders, recalls and other reminders or medical services. Our practice may also upload Shared Health Summaries. If you do not consent to this, please advise our reception staff.

  How can you lodge a privacy related complaint and how will the complaint be handled at our Practice?

We take complaints and concerns regarding privacy seriously. You should express any privacy concerns you may have in writing either by letter or email reception@boyupmedical.com.au. We will then commence the complaint process in accordance with our resolution procedure within a maximum of 30 days. Please include your mailing address and contact phone number.

You may also contact the Office of the Australian Information Commissioner (OAIC). Generally, the OAIC will require you to give them time to respond, before they will investigate.
