This is a fantastic opportunity for a part time Nurse (AHPRA registered) who is passionate about rural health, to join our friendly and professional team in delivering high quality, coordinated care to support the health and wellbeing of our community.
About the Practice
Boyup Brook Medical Services is an accredited, mixed billing practice, dedicated to providing high quality patient services and supporting community health and wellbeing.
Key Responsibilities (in liaison with the General Practitioners):
- Deliver high quality nursing services to our patients
- Prepare and conduct health assessments
- Develop and support the delivery of chronic disease management plans
- Assist in the facilitation of patient wellness clinics
- Collaborate with general practitioners and allied health professionals to provide integrated patient care
